    Learning Objectives: At the conclusion of the program, participants will be able to describe best practices for patient care in post-acute and long-term care, summarize clinical and practice updates as related to the post-acute and long-term care setting, and describe methods to support teamwork through inter-professional teams in the post-acute care long-term care environment that are designed to result in quality care and increased satisfaction of the residents, their family and the staff.

The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity.
